full time Family Practice at Horizon Health Network - Fredericton / Upper River Valley areas
Waterville Carleton CO, New Brunswick
Job ID: 18067

As a hospitalist, you will provide hospital-based, general medical care to inpatients with emphasis on post-admission (inpatient) management, efficient hospital stay, discharge planning in cooperation with the discharge coordinator, use of e-documentation, and effective communication with the patient and their family, and family physician.
At such time as accountability benchmarks are determined by the Health Authority or the Department of Health for Hospitalists, the benchmarks will become effective. In the meantime, hospitalist workload will be determined jointly by the Health Authority and the Physician.
You will participate in the after-hours in-patient/hospitalist on-call rota. There are opportunities for emergency shifts independent of the hospitalist rotation.

As a hospitalist:
• Provide medical care as the “most responsible physician” to inpatients including rounds, treatment, surgical assists if needed, and the planning of follow-up care. The total number of patients under one hospitalist’s care is not to exceed 15-20 patients, according to level of complexity.
• Assist with any in-hospital medical emergencies when on site as part of the code team, under the direction of the ER physician.
• Accept admission of transferred patients when their own family physician can’t be on-site to do so.

Generally:
• Participate in the after hours Inpatient/Hospitalist on-call coverage in coordination with other medical staff ensuring provisions are made for continual coverage of patient caseload during times when you are absent from your practice.
• Maintain all patient records current and in accordance with the Health Authority bylaws, medical staff rules, regulations, policies and procedures.
• Participate in the development, implementation, monitoring and evaluation of treatment, education, counseling and health promotion programs, disease and injury prevention and chronic disease management programs and services for individuals, families and the community.
• Participate in the use of departmental and corporate evidence-based rules of practice to ensure that expected standards of care are achieved.
• Participate in medical practice that is compatible with the Health Authority’s mission, values and strategic directions.
• Other duties falling within the scope of practice that may arise by circumstance could be assigned by the Chief of Family Practice.
Note: These responsibilities are the basic and prime duties associated with this position; other related and allied duties may be assigned from time to time as may be necessary to the operation of Horizon.
