Carrier Sekani Family Services (CSFS), Northern Health (NH) and First Nations Health Authority (FNHA) recognize the need for quality primary care services for First Nations communities in the Burns Lake area. The foundation of Carrier Sekani Family Services Primary Care Model is premised on nation rebuilding with an underlying philosophy that is grounded in Carrier and Sekani values and beliefs and based on an integration of services that supports a holistic approach to health. The next phase of development of the CSFS Primary Care model is to enhance the First Nations Primary Care Homes already developed and increase reliable and equitable service for the communities of Tachet, Wet’suwet’en, Woyenne, Burns Lake, Fort Babine, Cheslatta, Nee Tahi Buhn and Skin Tyee. CSFS physicians truly value the relationship-building aspect of the CSFS primary care model because it allows them to provide cultural safety and overcome historical barriers of disjointed medical care. With additional consultation time with patients, there is opportunity to address multiple factors that contribute to a patient’s health and wellbeing. To assist with our vision, a 1.0 FTE APP Service Contract for a full service family physician is available. In keeping with the value of providing services in underserviced areas where care did not previously exist, it is anticipated that the majority of clinical time will be spent on reserve enhancing access to primary care while developing and nurturing relationships and trust with community members. Additionally, it is envisioned that a CSFS physician presence will start to develop in the Lakes District Hospital where a large number of community members access acute care and diagnostic services. The projected split is 0.8 FTE primary care in community and 0.2 FTE acute care. To this end, the CSFS physician will work with the CSFS Medical Director, Northern Health Lakes Omineca Medical Director and the Lakes District Hospital Chief of Staff to establish a sustainable and progressive working relationship to advance acute and in-hospital care. CSFS physicians will visit the communities within the Burns Lake Area, including visitations to Lakes District Hospital and long term care settings following up on CSFS primary care patients. Recognizing the challenges of geography, the position will also provide support through CSFS virtual health services, utilizing a shared EMR accessible by the team from any CSFS location. The Lakes District Hospital and Health Centre is a 16 bed facility that opened in 2015. It sees an average of 12,000 ER visits per year. The hospital houses 13 acute care beds, 1 isolation room, 1 palliative care room, emergency services, diagnostic imaging and laboratory, as well as a primary care home with three full time physicians. Burns Lake services a local and surrounding population of approximately 13,000.The community has a strong sense of culture and incredible local talents.
