Attending Physician, Long Term Care – 1 Position Available *Independent Consultant– 1 Year Renewable Contract– Long Term Care

Royal Ottawa Place LTC is looking for an *Independent Consultant who can skillfully and competently perform the Services as an Attending Physician.

Overview of Positon:

Reporting to the Administrator of Royal Ottawa Place, this position provides a high level of knowledge, skill and experience needed in caring for medically complex residents in a climate of high expectations and strict regulatory requirements. The Attending Physician is accountable to the Medical Director for compliance with the homes policies, procedures and protocols for medical services. Primary responsibilities include reviewing of the residents’ care plans, including medications, treatments, rehabilitation therapy, nutritional interventions, nursing care, social work intervention and therapeutic activities to ensure care provided is resident centered focus.

The Attending Physician works collaboratively with other members of the interdisciplinary team to supervise the medical care of residents in his or her care, including participating in the assessment and care planning, monitoring changes in health status and providing treatments or interventions to meet the residents’ assessed needs.

The position implements strategies to maximize and improve the provision of resident centered medical care and promote inter- professional practice in the home. Strategies include providing medical care consistent with legal, regulatory requirements, evidence- based best practices and residents’ diagnosis, condition, prognosis and wishes, focusing on helping the resident attain his or her highest practicable level of functioning in the long term care environment.

Responsibilities:

• Dedicates a minimum of a ½ day per week at the long-term care home to provide services in collaboration with the interdisciplinary team (eg. Nursing, Pharmacy, Physiotherapy, etc.) including assessments.
• Reporting to the medical director, manages the care of assigned residents by consistently and effectively applying the medical care

delivery process, including recognition, problem identification, diagnosis, goal identification, intervention and monitoring progress. • Shall be on-call 24 hours per day, year round for the residents of the long-term care home or shall ensure on-call coverage by a back-up physician and must also provide after-hours coverage, on- call coverage and vacation coverage. • Works collaboratively with psychiatric services to meet the needs of the residents in the home that have a stable /chronic mental health condition. • Provides care that uses resources discreetly and minimizes unnecessary discomfort and disruption for residents • Identifies rationale for and uses key resident databases (for example, the Minimum Data Set (MDS)) in care planning, facility reimbursement and monitoring of quality. • Addresses resident care needs, visits, phone calls and documentation in an appropriate and timely manner. • Identifies circumstances in which palliative and/or end of life care may benefit the resident and family members and work collaboratively with members of the interdisciplinary team. • Works effectively with members of the interdisciplinary team including the medical director, in providing care based on understanding and valuing the general roles, responsibilities and levels of knowledge and training for those of various disciplines. • Supports resident’s discharge and transfers by following-up with receiving physician or health care practitioner as needed after the transfer of an acutely ill or unstable resident. • Conducts residents’ medication drug reviews quarterly or often as needed and document review in residents’ health record. • Participates in scheduled admission, annual and incidental care conferences as part of the inter-professional team.

Qualifications:

• Must be a legally qualified medical practitioner licensed to practice medicine in the Province of Ontario and at all times be a member in good standing of the College of Physicians and Surgeons of Ontario (CPSO) and hold an acceptable certificate of registration to practice medicine issued by the CPSO under the Medicine Act and must provide proof of same annually.
• Membership with the Canadian Medical Protective Association
• Experience working in Long Term Care (LTC) setting is an asset
• Having courtesy privileges at a local active treatment hospital is an asset.
• Knowledgeable in the Long Term Care Homes Act, 2007, Ontario Regulation 79/10, and the compliance inspection protocols required.
• Knowledge of the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI), PointClickCare (PCC) and all associated quality reports.
• Demonstrated ability to be adaptable, build trust, and provide coaching and mentorship, facilitate change and continuous improvement.
• Demonstrated skills to effectively interact with staff, residents and families by using appropriate strategies to address sensory, language, health literacy, cognitive and other limitations.
• Ability to utilize data (for example, Quality Reporting System Indicators, and MDS data, etc.) to make informed decisions and to improve care and services provided to residents.
• Skills in Microsoft Office including Excel, Power Point, Access and Word.
